Job Title: Case Manager Profession: Registered Nurse Specialty:

(08:00 - 16:30) Experience: Minimum 2 years of acute care case
management/discharge planning experience required License:
California state license required; pending license accepted but
must be licensed upon start date Certifications: Relevant nursing
certifications preferred Must-Have: Previous charge experience Must
have experience with patient ratios and charting systems. Competent
with Computerized documentation and InterQual Criteria.
Description: Start date: ASAP. Floating expectation: No. First-time
travelers must be screened. Patient ratios during the day: 1:20.
Weekend requirement: Every other weekend. Must be available for
holidays. Experience with care coordination and discharge planning
is essential. Knowledge of adoption/foster care, drug/alcohol abuse
treatment options, and mental health is preferred. Ability to
organize and participate in family meetings is required. Experience
with various insurance programs is necessary. Competent in
arranging durable medical equipment and transport services
independently. Travelers may be expected to float between
facilities within the state. All travelers must commit to working
on three of the five holidays. References will be required. A
minimum of 2 supervisory references covering 1 year of employment
is essential. Experience with InterQual guidelines is highly
preferred. Acute behavioral health experience does not meet the
acute hospital requirement. Knowledgeable in Medicare guidelines.
Assessing patients, care coordination, and discharge planning
experience is crucial. Must be able to manage a caseload of up to
25 patients. Participation in the development of treatment plans is
